DETROIT MIDDLE SCHOOL WIRELESS LAB CHECKOUT
THIS LAB IS FOR MIDDLE SCHOOL AND HIGH SCHOOL CLASSES. PLEASE RESERVE THE LAB EARLY ENOUGH TO GUARANTEE YOUR SPOT. YOU CAN ONLY CHECK OUT THE LAB FOR TWO CONNECTIVE DAYS. THE LAB MUST BE RETURNED TO MRS. NICHOLS IN THE HIGH SCHOOL LIBRARY AT THE END OF EACH DAY. ALL COMPUTERS WILL BE SHUT DOWN AND PLACED BACK IN THE CART BETWEEN CLASSES, AND DURING LUNCH PERIODS. PLEASE THE RULES BEFORE USING THE LAB.
Please complete the form below to reserve the wireless lab. Please read the general wireless rules below this form before submitting. This form will be submitted to Mrs. Nichols. Mrs. Nichols will email you with a confirmation of your requested time and date. Please check check the current reservations before submitting. Teachers will be given a teacher agreement and a student agreement. Teachers will need to sign the teacher agreement, and make enough copies for each of your students to sign the student agreement. The teacher and student agreements will only need to be filled out once each school year, not every time you use the lab. These agreements will be completed prior to taking the computers out of the lab cart. Please get with Mrs. Nichols at least an hour before you checkout the lab to copy and have the agreements signed, so your lab time will not be taken up. If any teacher of student does not agree to the agreement, they will not be allowed to use the lab until an agreement is signed. If for some reason, a teacher or student is banned from the lab, only Mrs. Looney, the Middle/High School Principal, Mr. Horne, Technology Director, and Mr. Lyon, Superintendent, has the authority to allow them access again. Mrs. Nichols has the authority to stop the lab use to any teacher or student not following the Lab rules.
TEACHERS AND STUDENTS: The first year of the lab has gone well. We thank you for taking care of the computers, and the mobile cart. We have not had any problems concerning the lab due to misuse or neglect.

USE AND CARE POLICY
GENERAL RULES
The Detroit Middle School wireless computer lab is provided to Students and Teachers on the Detroit Middle School Campus. The lab will be managed and monitored by the Detroit High School Librarian, Mrs. Nichols. Mrs. Nichols will keep a schedule of the lab use. Any teacher needing the lab must contact Mrs. Nichols and set up a day and time when they will need the lab. Teachers will be responsible for the care and use of the lab, and will sign an agreement stating they understand the use and the care of the lab. The lab computers print to the Network printer in the Middle School Teachers work room.
Only classroom teachers may check out the lab. No substitutes or Aides my check out the lab. If a teacher has the lab reserved, and is planning not to be at work, (Workshop or sick), they must notify Mrs. Nichols either directly or via their Principal or Secretary.
RULES:
Please reserve the lab early as possible to avoid your time and date being taken. I suggest you reserve the lab at least a week in advance. High School Teachers may use the Middle School Wireless Lab providing availability. The Lab may only be reserved for a maximum two consecutive days per teacher/class. If Mrs. Nichols is absent the day you are scheduled to have the lab, contact Mr. Horne for access to the Lab.
